We understand that today’s fast-paced world creates unprecedented challenges for family members and loved ones who want to stay informed about our in-home care clients. That’s why we’ve introduced a unique service where clients, their family and caregivers can communicate anytime, from anywhere about the health and well-being of our patients.

When you sign up for in-home care through Unified Home Care, you will be given an opportunity to choose this service. If you do elect to participate you will have complete control over who has access to your online account, which is only available through a secure log-in feature.

Once your account is set up, your Unified Home Care caregiver will post information such as the date of their visit, vital signs, overall physical and mental status, or other general observations. Those who have access to the account can then review this information and reply to the caregiver with questions or comments.

This feature is particularly helpful to family and friends who live elsewhere, travel frequently, or due to schedule constraints are not able to visit as often as they would like. It’s a great way to stay actively involved in your loved one’s care even if you are not able to be physically present in their home.
